Get an Electronic or Paper Copy of Your Medical Record

You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you.

We will provide a copy or summary of your health information, usually within 30 days of your request. A reasonable fee may apply.

Ask Us to Correct Your Medical Record

You can request corrections if you believe your record is incorrect or incomplete. If we deny your request, we will explain the reason within 60 days.

Request Confidential Communications

You may request that we contact you in a specific way such as:

• home phone
• office phone
• email
• alternate mailing address

We will honor all reasonable requests.

Request Restrictions on Information Sharing

You may request limits on how your information is used or shared for treatment, payment, or healthcare operations.

If you pay for a service out-of-pocket in full, you may request that we do not share that information with your health insurer.

Request a List of Disclosures

You may request a list of disclosures made during the previous six years. One request per year is free.

You may request a printed copy of this notice at any time.

Choose Someone to Act for You

If you have granted medical power of attorney or legal guardianship, that person may exercise your rights.

File a Complaint

If you believe your privacy rights have been violated, you may contact us directly or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.

We may use your health information and share it with other professionals involved in your care.

We may use your information to manage treatment plans, improve care, and contact you when necessary.

Billing for Services

We may share information with insurance companies or billing entities to receive payment for services.

We will never share substance abuse treatment records without written authorization.

Washington law may allow minors to consent to certain healthcare services without parental approval.
